Learning Objectives:

To recognize extrahepatic complications following liver ablations

To understand the underlying causes and preventative strategies for these complications

Background: With the growing use of liver ablation as standard care for primary and secondary hepatic tumors, the incidence of extrahepatic complications associated with thermal ablation procedures has increased globally. Thermal injury from ablation can lead to a unique spectrum of complications affecting adjacent organs

Clinical Findings/Procedure Details: This presentation will illustrate, through case examples, the various manifestations of extrahepatic injuries resulting from liver ablation. Case highlights include diaphragmatic perforation with bilious effusion, diaphragmatic hernia, colonic perforation, stomach perforation, chest wall bleeding, needle tract seeding, pneumothorax and pericardial injury. Preventive techniques such as artificial ascites creation to protect the diaphragm, hydro-/pneumodissection, and interventional displacement strategies (e.g., balloon interposition between the liver and adjacent viscera) will be demonstrated with clinical cases.

Conclusion and/or Teaching Points: Attendees will gain practical insights into identifying at-risk organs, recognizing patterns of thermal injury, and implementing effective techniques to prevent extrahepatic complications during liver ablations. Early recognition and adoption of preventive measures are crucial to improving patient outcomes and reducing morbidity.
