219 - Coiling Middle Meningeal Artery for Embolization of Subdural Hematoma in Cancer Patients: Safety and Efficacy

Purpose: To evaluate the safety and efficacy of coil-only middle meningeal artery (MMA) embolization for the treatment of chronic to subacute subdural hematoma (SDH) in patients with cancer.

Materials and Methods: A single-center retrospective analysis was performed on 34 cancer patients who underwent 49 MMA embolizations using coils alone, including 15 bilateral procedures (44%) for SDH between 2022 and 2025. Clinical and radiographic outcomes were evaluated, including hematoma resolution, change in SDH thickness, need for reoperation, and overall survival.

Results: At final imaging follow-up (median: 98.5 days [IQR: 22–179]), 34 hematomas (74%) demonstrated reduction in thickness, 26 (57%) showed ≥50% reduction, and 17 (37%) achieved complete resolution. No statistically significant difference was observed in the magnitude of SDH thickness reduction (mean difference: 4.1 mm greater reduction in unilateral group; 95% CI: –1.5 to 9.7 mm; p = 0.14) or in SDH resolution rates (OR: 0.77 for bilateral vs. unilateral; 95% CI: 0.23–2.58; p = 0.67) between unilateral and bilateral MMA embolization. Two patients (6%) required surgical evacuation after MMAE. Twenty patients (59%) died due to cancer progression during follow-up. Median survival was significantly longer in patients with reduced hematoma thickness on initial follow-up imaging compared to those without improvement (331 vs. 21 days, p < 0.001).

Conclusion: Coil-only MMAE may be a safe and effective treatment option for SDH in patients with cancer. Hematoma improvement was associated with prolonged survival, supporting the clinical relevance of embolization in this high-risk population.
