324 - A Single Center Preliminary Experience of Percutaneous Hepatic Perfusion with Melphalan for Liver Metastases of Uveal Melanoma

Purpose: To evaluate the safety, feasibility, and preliminary efficacy of Percutaneous Hepatic Perfusion (PHP) for liver metastases of uveal melanoma in a single-center retrospective analysis. The primary aim is to assess objective response rates (ORR), disease control rate (DCR) and characterizing procedure-related adverse events.

Materials and Methods: A retrospective review of patient records was conducted for all patients with liver metastases from uveal melanoma who underwent PHP at our institution from March 2024 to September 2025. A total of 13 patients with 54 PHP intervention included. There were 5 (38.5%) females and 8 (61.5%) males, and median (IQR) age was 67 (10). Out of 13 patients three completed 6 interventions, five are continuing, three stopped early due to disease progression and two patients opted not to continue with treatment. Adverse events and complications were graded according to the Common Terminology Criteria for Adverse Events (CTCAE). Descriptive statistics were used to summarize all patient and procedural data.

Results: ORR of 61.5% (95% CI: 35.5-82.3%) including one patient (7.6%) with a complete response. DCR was 76.9% (95% CI: 49.7-91.8%). One patient died with disease progression. Among the first 13 consecutively treated patients at our center, 10/13 had at least one target tumor volume reduction. Complications include thrombocytopenia (n=29, 53.7%), anemia (n=21, 38.8%), fatigue (n=12, 22.2%), leukopenia (n=10, 18.5%), neutropenia (n=7, 12.3%), ALP increase (n=3, 5.5%), elevated ALT (n=3, 5.5%), chemical cholecystitis(n=2, 3.7%), bilirubin increase (n=1, 1.8%), pain (n=1, 1.8%), elevated AST (n=1, 1.8%), edema (n=1, 1.8%) without any intraoperative complications.

Conclusion: This preliminary single-center experience with PHP for liver metastases of uveal melanoma demonstrates better ORR than other treatment modalities for metastatic uveal melanoma.
