Foam Sclerotherapy–Associated Migraines: Insights into Pathophysiology and Procedural Modifications

Learning Objectives: To evaluate the association between sclerotherapy and post-procedural migraine symptoms, discuss proposed pathophysiologic mechanisms, and provide clinical guidance for risk stratification and procedural modifications.

Background: Foam sclerotherapy is a widely used, minimally invasive treatment for varicose veins. While typically well-tolerated, neurological symptoms such as migraine with aura have been increasingly reported following the procedure. Proposed mechanisms include foam microembolization to the cerebral vasculature via right-to-left shunts such as a patent foramen ovale (PFO), triggering cortical spreading depression — the physiologic correlate of migraine aura. Patients with a pre-existing history of migraines, especially with aura, appear to be at higher risk. Despite a growing number of case reports and small series, this complication remains under-recognized among providers performing venous interventions.

Clinical Findings/Procedure Details: This exhibit reviews cases and published literature describing migraine-like symptoms after foam sclerotherapy, with onset ranging from minutes to hours post-procedure. Common symptoms include visual aura, headache, photophobia, and nausea. Diagnostic evaluation, including neurologic workup and echocardiography with bubble study, is discussed for patients with recurrent or severe symptoms. Procedural factors such as foam type (air vs. carbon dioxide-based), injection volume, patient positioning, and inadvertent entry into the deep venous system are analyzed. Imaging, procedural technique, and post-procedure monitoring are reviewed, with emphasis on identifying patients at elevated risk.

Conclusion and/or Teaching Points: Migraine symptoms following foam sclerotherapy represent a clinically relevant but underappreciated complication, particularly in patients with known migraine history or undiagnosed PFO. Understanding the pathophysiology and patient risk factors enables clinicians to better counsel patients and adopt risk-reduction strategies. Procedural modifications—such as limiting foam volume, using carbon dioxide-based foam, and avoiding deep venous communication may help mitigate embolic events. Recognition of post-sclerotherapy migraine allows for appropriate evaluation and management, reinforcing the importance of multidisciplinary awareness among interventionalists, neurologists, and cardiologists.
