768 - Targeted CT-Guided Epidural Blood Patch for Complex Postoperative CSF Leaks

Background: Postoperative CSF leaks occur in ~0.5–16% of spine surgeries and can lead to intracranial hypotension, pseudomeningocele, and cord compression. Conservative measures may fail, prompting minimally invasive alternatives. Epidural blood patching provides a transient pressure effect and dural sealing. In compromised canals, even small epidural volume loads can produce disproportionate thecal pressure spikes and spinal cord perfusion pressure (SCPP) decline. A pressure-balanced technique mitigates these pressure spikes while achieving a seal.

Clinical Findings/Procedure Details: This educational exhibit will discuss the clinical indications which include persistent symptoms and recurrent extradural collections. It will also present annotated case images demonstrating this pressure-balanced CT-guided EBP technique. Pre-procedure panels demonstrate the postoperative extradural collection, preprocedural appearance of the spinal canal and cord. Intraprocedural CT images taken during CT guidance. A lumbar puncture is shown establishing intrathecal access for confirmatory myelography, canal visualization, and pressure equilibration. Sequential images illustrate dual epidural needle placement into the collection: limited passive drainage through one needle gradually decompresses the collection to create space, while slow autologous blood injection through the contralateral needle proceeds under intermittent CT checks. Throughout, lumbar access is maintained to buffer pressure changes. A consolidated figure highlights the pressure-management sequence: equilibrate through CSF lumbar access to explicitly protect SCPP, decompress with limited drainage, then inject slowly under intermittent CT monitoring. Post-procedure panels display improved spinal canal narrowing on follow-up imaging.

Conclusion and/or Teaching Points: A pressure-balanced, CT-guided dual-needle EBP minimizes abrupt thecal pressure rises, protecting spinal cord perfusion while reliably sealing extradural CSF collections.

Standardized steps and simple outcomes (neurologic exam, canal diameter, retreatment rate) enable objective validation and safer adoption.
