Purpose: To investigate the long-term outcome of vascular malformations of the lip treated by combined endovascular and plastic surgery.

Materials and Methods: All patients treated between 2000 and 2023 for vascular malformation of the lip were included. Demografics, symptomatology and type/extension of the vascular malformation were analyzed. Treatments were done by sclerotherapy/embolization and if required plastic surgical reconstruction. Treatment complications and fysical/psychological outcome (by questionaire) were analyzed.

Results: Thirteen patients (10 females, mean age 27.5 (1-53) years) were included. Protrudent swelling in all, discoloration (N=10), heart synchrone pulsations N=5), pain N=2), bleeding (N=2) and saliva loss, were the referring reasons. Eight patients suffered from a venous malformation treated by direct sclerotherapie with polidocanol, 5 from an arteriovenous malformation treated by direct or transarterial embolization with glue. Location was upper lid in five. Four patients (2 venous and 2 arteriovenous) underwent supplementary surgery with direct pre-operative glue embolization. No major post-operative complications. Long term outcome (in total 48 treatment in a mean time of 3.7 years), observation time (mean 5.1 year) of the physical aspect was a strong improvement, psychologically the impact clearly reduced and functionally also clear improvement.

Conclusion: Vascular malformation of the lip can be treated successfully and safe by sclerotherapy/embolization Mutliple treatment sessions, and in the event of severely deformation of the lip, a combination with reconstructive surgery after preoperative embolization is an excellent option.
