Purpose: Yttrium-90 transarterial radioembolization (Y90) is established for hepatocellular carcinoma, but its role in hepatic adenomas is undefined. This case series evaluates the safety and efficacy of Y90 in hepatic adenomas.

Materials and Methods: We retrospectively reviewed all patients treated with Y90 for hepatic adenomas at a tertiary center from 2010–2024. Clinical, laboratory, imaging, and safety follow-up data were collected. Primary outcomes were CTCAE adverse events (AE) and long-term safety; secondary outcome was overall radiologic response.

Results: 5 patients (mean age 39.2 years, range [13.6–61.5]) with 9 unresectable lesions (mean 1.8 per patient [1–4]) were treated. 8/9 lesions were diagnosed via biopsy, 1/9 by imaging. Indications for Y90 included lesion size >5 cm in all female patients of child-bearing age at risk of hemorrhage (3/5) and higher risk of malignant transformation in male patients (2/5). Mean lesion size was 6.0 cm [2.5–9.5]. All treatments were performed as radiation segmentectomies with targeted absorbed dose >205 Gy. Grade 1 AEs (nausea, fatigue, anorexia, diarrhea) occurred in 3/5 patients; no grade 2 or greater AEs occurred. 3/5 patients had follow-up laboratory values, none of whom had evidence of liver dysfunction on labs measured by ALT or AST > 2x upper limit of normal (ULN), ALP > 2x ULN, or total bilirubin > 2 mg/dL. No patients developed evidence of liver cirrhosis, fibrosis, or portal hypertension on latest MRI. mRECIST Objective response rate (ORR) was 100% (complete response [CR]: 6/9 [67%], partial response [PR]: 3/9 [33%]). Mean time from Y-90 to follow-up MRI was 7.1 weeks [6.1–25.7], to latest labs was 14.5 months [4.4-22.3], and to latest MRI was 12.5 months [1.6-21.9]. One patient underwent surgical resection post Y-90. No lesions recurred.

Conclusion: This single-institution series shows Y90 radioembolization can be performed safely for hepatic adenomas with high objective response rates.
