Purpose: To compare quality of life in women with venous-origin chronic pelvic pain (VO-CPP) to US normative populations (NP) and published cohorts with endometriosis (ENDO).

Materials and Methods: The SF-36 questionnaire was completed by patients in the concept elicitation phase of an SIR Foundation–funded project to develop a PROM tool for VO-CPP {1}. Participants were menstruating women ≥18 years with VO-CPP >6 months, recruited by gynecologic pain specialists. After a complete evaluation, the specialist deemed VO-CPP to be the most significant diagnosis; all had imaging confirming pelvic varicosities.

Mean SF-36 T-scores (±SD) from women with VO-CPP were compared with NP (defined as mean=50, SD=10) using one-sample t-tests, and with published scores for women with ENDO using Welch’s two-sample t-tests {2}. Effect sizes (Cohen’s d) were calculated to quantify the magnitude of T-score group differences.

Results: The VO-CPP cohort (n=21) scored significantly lower in all SF-36 domains (p < 0.05) compared to NP (Cohen’s d = -0.6 to -1.1, indicating moderate–large reductions). Compared to ENDO, VO-CPP patients showed similar scores in several domains, and significantly worse in physical role limitations, pain, vitality, and the physical component summary scores (d = –0.6 to –1.0). Results shown in Table 1.

Conclusion: VO-CPP patients reported moderately to markedly lower health scores than NP in all SF-36 domains, and similar or worse scores than women with ENDO. These findings demonstrate the need to include VO-CPP in women’s health policy and research priorities.
