Purpose: To review over 20 years of a single-center experience with elective transcatheter coil embolization of unruptured splenic artery aneurysms (SAA).

Materials and Methods: A retrospective review of unruptured SAA electively treated with transcatheter coil embolization at a single-center from October 2002 to June 2025 was performed. Patient records and imaging were reviewed to obtain demographics, SAA number and size, procedural technique, and technical/clinical outcomes. Primary technical success (cessation of flow into aneurysm inflow vessel and no evidence of contrast opacification of the aneurysm sac), secondary technical success (freedom from aneurysm opacification on follow up imaging), reintervention rates, adverse events, 30-day mortality rate, and freedom from aneurysm rupture were recorded.

Results: A total of 153 unique aneurysms were treated in 127 patients. The mean age was 54.7 years (range, 24-89), and 85 patients (66.9%) were female. Mean aneurysm size was 24 mm (range, 5-80). All SAA were electively treated with transcatheter coil embolization. Twelve (7.8%) required an adjunctive technique or embolic [stents (4), plugs (3), gelfoam (2), Onyx (2), glue (1)]. Primary technical success was achieved in all but one patient (99.2%). Median follow up was 25.8 months (range, 1 day-19.9 years). Secondary technical success was achieved in 122 patients (96.0%). Reintervention was required in four patients (3.1%), all of whom remained successfully treated following reintervention. Major splenic infarction occurred in 17 out of 127 patients (13.4%), including nine out of 27 patients (33.3%) with portal hypertension and eight out of 100 (8.0%) without portal hypertension. Thirty-day follow up was available for 105 patients (82.7%). One mortality (0.8%) occurred within 30 days due to transfusion reaction, unrelated to SAA embolization. There were no cases of SAA rupture in the follow up period.

Conclusion: This large single-center experience demonstrates that elective transcatheter coil embolization is a safe and effective treatment for unruptured SAA with high rates of technical success and freedom from aneurysm rupture.
