Influence of Sarcopenia and Myosteatosis on Outcomes from Lung Ablation

Purpose: To evaluate how sarcopenia and myosteatosis can predict the outcome of lung cancer patients who underwent lung ablation

Materials and Methods: In this IRB-approved retrospective cohort study, patients with primary or metastatic lung cancer who underwent lung tumor ablation between January 2014 and March 2023 were assessed for sarcopenia and myosteatosis. Semi-automated thresholding was used to measure muscle volume and density at the L3 vertebral level in pre-treatment CT scans performed within one year prior to ablation. Sarcopenia (SMI < 50 in men, < 39 in women) and myosteatosis (muscle CT attenuation < 41 HU for BMI < 25, < 33 HU for BMI ≥25) were categorized according to standard criteria. Associations with post-ablation survival were assessed with Kaplan-Meier analysis.

Results: A total of 95 patients met inclusion criteria, 51 males and 44 females, median age of 66 years (median interquartile range: 56-74 years). Within the cohort, 51% met sarcopenia criteria, and 68% exhibited myosteatosis. Kaplan-Meier survival analysis revealed a significant difference in survival rates based on myosteatosis (log-rank p-value = 0.0013) and sarcopenia (log-rank p-value of 0.0034), with the non-sarcopenic and non-myosteatosis group demonstrating superior survival outcomes

Conclusion:

Sarcopenia and myosteatosis as a predictive factor for treatment outcomes in these patients.
