Four 48-well plates were seeded with HepG2, a human hepatocellular carcinoma cell line, at a concentration of 10,000 cells per well. After 24 hours of incubation, the plates were irradiated with 0, 2, 4, and 6 Gy irradiation using a clinical linear accelerator (Elekta Versa HD, beam energy of 6MV, dose rate of 5Gy/min). In each well plate, three wells in the Lipiodol group had a 10-mm cylindrical Lipiodol in tailor-made well-insert placed 3.39-mm above the cell layer, and a 17.78-mm Lipiodol-filled well was placed 0.19-mm below the cell layer during the irradiation. Other three wells in the control group, water was placed in the same manner instead of Lipiodol. After 96 hours of incubation post-irradiation, cells were stained using a caspase-3 assay kit (NucView 488, 2.0 mM) to detect apoptosis and with cell nuclei stain (NucSpot 650), then analyzed by fluorescence microscopy (Celldiscoverer 7, ZEISS). Four ROIs were set for each well, and the areas of apoptosis and cell nuclei within these ROIs were measured. The percentage of apoptosis to cell nuclei was calculated and compared between Lipiodol and control groups using the Mann-Whitney U test.
Results:
The percentage of apoptotic cells in each subgroup (0, 2, 4, and 6 Gy, respectively) was as follows: control group, 0.1, 14.8, 47.3, and 51.5%; Lipiodol group, 0.2, 43.2, 59.7, and 73.0%. Statistical analysis demonstrated significant differences at 2 Gy (p < 0.01), 4 Gy (p = 0.0387), and 6 Gy (p < 0.01), whereas no significant difference was observed at 0 Gy (p = 0.319).
Conclusion:
The presence of Lipiodol in close proximity to target during radiation therapy suggested a potential increase in local therapeutic effect.
