Purpose: 1. To prospectively explore the feasibility of performing percutaneous CT-guided lung biopsies using a table-mounted needle-guiding robotic system with steering capabilities under local anesthesia.
2. To retrospectively compare radiation dose to a patient cohort of free-hand percutaneous lung biopsies

Materials and Methods: The study included 25 patients (12/13 male/female; mean age 73±6.65 years) who underwent robotic assisted lung biopsy under local anesthesia. Feasibility was defined as the distance of ≤ 10 mm between needle tip after insertion and the nearest lesion edge on the CT scan images. Evaluated parameters included accuracy of needle placement, trajectory length, procedure duration, number of acquired computed tomography scans robot-related troubles as well as adverse events.

Results: Biopsy needle insertion was feasible in all cases (100% technical success rate). The mean diameter of target lesion was 24±15mm whilst the mean distance from pleural surface was 25±18mm. The mean distance from the skin surface was 69±30 mm. Median scan number was 4 and the mean time from the first to final scan was 16.9±5.6 minutes. Neither robot-related complications nor adverse events occurred. Diagnostic yield was 92% (23/25 biopsy cases). Compared to a patient cohort of free-hand percutaneous lung biopsies both computed tomography dose index (CTDI) and dose-length product (DLP) were significantly lower in the robotic arm (p-value < 0.025 and p-value < 0.001 respectively).

Conclusion: Percutaneous computed tomography-guided lung biopsy using a table-mounted robotic system was feasible and demonstrated high technical success, adequate diagnostic yield, and safety profile.
